GLP-1 use came with a 14 percent reduced risk of substance use disorders in 600,000 veterans

People with type 2 diabetes who took a GLP-1 medicine had a 14 percent reduced risk of developing a substance use disorder compared with people on another diabetes treatment, across more than 600,000 US veterans followed for three years. The steepest single drop, 25 percent, landed in opioid use disorders.
What makes the result unusual is that it held across every substance examined rather than in one lane, and the pattern showed up in the first year and was still there in the third. Among veterans who already had one, starting a GLP-1 was associated with 39 percent fewer overdoses and a 50 percent reduction in drug-related deaths. GLP-1 receptors sit in the brain circuits that handle reward and motivation, and the lead author calls the effect a quieting of drug noise.
Who was in the dataset decides how far this travels. The population was largely older white male veterans, and the analysis compared people already on one medicine with people on another rather than assigning treatment at random. Researchers did not test doses or compare the drugs against each other, so what is still unknown is which molecule, at what dose, does any of this. A multisite trial in opioid use disorder is now running, and any decision belongs with a qualified clinician who knows the health history.
