Tirzepatide kept nearly 99% of prediabetes patients diabetes-free over three years
One new case prevented for every nine people treated, for as long as the ninth one keeps filling the prescription.

Tirzepatide cut the risk of progression to type 2 diabetes by 94% against placebo over three years, and nearly 99% of people taking it were still diabetes-free at the end of the 176-week treatment period. That is one new case prevented for every nine people treated, in a Phase 3 trial of 1,032 adults with prediabetes who were obese or overweight.
Prediabetes is where the arithmetic changes. The comparison is not another drug but the ordinary course of the condition, and everyone enrolled was already on a reduced-calorie diet and more physical activity, so the 94% sits on top of those changes rather than instead of them. Full results were published in a peer-reviewed medical journal, and the number is pooled across the 5 mg, 10 mg and 15 mg doses, so it describes a group rather than a prescription.
Stopping is where the picture thins. At the 193-week mark, after a 17-week stretch off treatment, 2.4% of people who had taken tirzepatide had been diagnosed with type 2 diabetes against 13.7% of the placebo group, and the company's own framing is that long-term therapy is what maintains the result. Prevention that lasts as long as the prescription is an open-ended cost, and who it is worth paying belongs with a qualified clinician who knows a person's health history.
