NICE endorsed tirzepatide for obesity, on a narrower group than Lilly asked for

NICE endorsed tirzepatide for adults managing overweight and obesity alongside changes to diet and exercise, in draft guidance issued Tuesday. That opens the way for England's National Health Service to cover it, but only for people starting at a body mass index of at least 35 kg/m2 who also carry a weight-related condition.

Lilly had proposed cover from a body mass index of at least 30, and the watchdog rejected that as an inefficient use of NHS resources. Behind the coverage decision sits evidence that the drug plus changes in diet and exercise beat diet and exercise alone, which is the comparison a public system actually pays against.

Cost is where the enthusiasm meets the ledger. The watchdog flagged a high level of uncertainty in its own cost-benefit calculus, and a Cambridge metabolic researcher noted that while drugs like this reduce the risk of expensive complications such as type 2 diabetes and heart attacks, their cost lands as an immediate financial pressure while NHS budgets are tight.

Patients inside the narrower criteria are who this reaches, and the gap between what guidance permits and what a system can absorb is the part left open. It is the second endorsement for tirzepatide here, after a type 2 diabetes sign-off in September.
